3.12 Medications • Assess: 1.

Determine learner need(s) related to assigned medication(s) 2. Review pertinent patient data associated with assigned medication(s): a) Perform vital sign measurement prior to preparing medication when indicated b) Measure vital signs using proper technique c) Measure vital signs accurately (within established parameters)  Measure body temperature  Count pulse: apical or radial, based on patient’s current clinical condition  Measure manual blood pressure as indicated  Measure pain level using one of the methods below based on patient’s current clinical condition: • Numeric Rating Scale • FACES Pain Rating Scale • FLACC Pain Assessment Tool • Observed behaviors d) Laboratory values when indicated 3. Inspect site of administration prior to medication administration (e.g., condition of skin, intravenous insertion site, tube patency, ability to swallow) • Implement: 1. Provide education based on learner needs 2. Identify indication for use of each medication in relation to patient’s current clinical condition 3. Verbalize correct decision whether the medication(s) can be administered or need to be withheld based on pertinent patient data 4. Prepare the assigned medication(s) for administration: a) Select the prescribed assigned medication(s) using the MAR b) Prepare medications using proper technique c) Measure the prescribed dosage(s) d) Label container with drug name, strength, and amount if medication is unidentifiable 5. Verbally identify the patient by comparing two of the following pieces of information on the patient’s identification band against the MAR immediately before administering medication: a) Patient name b) Date of birth c) Medical record number 6. Administer medication (for all medications): a) Position patient to facilitate medication administration
