Chapter 5: Evidence-Based Practice Assessment Tools Evidence-based practice guidelines and assessment tools are incorporated within the Critical Elements of the competencies and are integral to nursing care provided during PCSs. Listed below are the tools students will use during a PCS. Students are expected to be proficient in the use of each tool during the CPNE testing period. The tools included in this section are: • Braden Scale for Predicting Pressure Sore Risk: Used in skin management • Comfort Daisies Scale: Used in comfort management • Comfort Verbal Rating Scale: Used in comfort management • FACES Pain Rating Scale: Used with administering medications and vital signs • FLACC Pain Assessment Tool: Used with administering medications and vital signs • Morse Fall Scale: Used with musculoskeletal management • Numeric Rating Scale: Used with administering medications and vital signs
