N104: Essentials of Nursing Care - Health Safety

N104: Essentials of Nursing Care – Health Safety Study Guide o The goals and expected outcomes may have been unrealistic for thepatient. o The nursing interventions may have been inappropriate for the patient’scondition. o The patient’s status or health condition may havechanged. • Finally, the plan of care is adjusted so that interventions that are working are reinforced and amplified, and interventions that are not working are adjusted or terminated. Although commonly thought of as the final step in the nursing process, evaluation simply completes the cycle and allows the nurse to again perform the assessment, diagnosis, planning, and implementation phases with more complete knowledge of the patient’s reactions to the plan. Evaluation must be done as early as possible after interventions are done in order to ensure that the plan of care can be quickly revised if goals or interventions are determined to be inappropriate. Evaluation must also be part of the documentation in the patient record and in the hand-off report given to other members of the healthcare team. 1.6 Conclusion The nursing process is the basis for all nursing care provided to a patient or client in any setting. Because it is grounded in the scientific method, the nursing process enables the healthcare team to provide patient-centered care using constantly updated data. The cycle of assessment, diagnosis, planning, implementation/intervention, and evaluation ensures that the data collected and used in a patient’s care are constantly refined, resulting in goals tailored for the patient and in improved patient outcomes.
