Organ Donation Organ donation is an important issue in nursing, facilitated by the Uniform Anatomical Gift Act of the United States , as discussed earlier in this chapter. Living people can choose to donate one or more of their bodily parts, and surviving spouses or relatives can also donate organs after the loss of a loved one if they haven't made a decision. Nurses play a vital role in ensuring that clients 18 years of age or over have an advance directive which includes information on organ and tissue donation preferences in their medical records. In the event that the client does not have an advance directive, the nurse can provide information about how to obtain an advance directive as well as information regarding organ donation. A specialized nurse, known as a procurement nurse , coordinates the harvesting and collection of the organs. Some healthcare organizations will have staff nurses approach the family regarding donations. Those nurses must receive specialized training per the Federal Conditions of Participation . Furthermore, the nurse should ensure that donation documents are complete and approved . By doing these things, nurses can help ensure that the client's wishes are honored. C. Client & Continuity of Care Continuity of care is an essential concept in nursing. Nurses are responsible for helping provide a seamless experience for clients, working together with other healthcare providers to provide high-quality, cost-effective care. To ensure continuity of care, nurses must be familiar with proper procedures for admissions , transfers , and discharges , as well as securely transferring client information from one agency to another. When transferring clients, nurses should also follow up on any unresolved issues and provide reports on assigned clients. Furthermore, nurses should ensure that all necessary forms are completed accurately and that clients are regularly updated on their progress so that they remain informed and involved in their care. In closed-loop communication , the person receiving the information repeats it back to ensure the message is understood correctly.
