N104: Essentials of Nursing Care – Health Safety Study Guide Evaluation As with all nursing care, the evaluation phase is ongoing throughout the process. Keeping patients free from nosocomial infections or injuries (infections and injuries that are a result of treatment in the hospital) should be an ongoing priority. Frequent contact with and assessment of the patient and patient environment will result in timely adjustments to the patient's care plan. The reassessment of the patient should be compared to the expected outcomes on the care plan to determine progress or lack of progress. When the patient is not making progress toward the expected outcomes related to biological safety, the nurse must ask the following questions: • Are the goals realistic? Should the goals be more short term? • Is the patient able to do the things outlined in the plan of care, or does he requiremore assistance? • Were the teaching efforts effective? Does the patient understand her disease and the treatment being provided? • Has the patient's condition changed, thus requiring a change in the plan? • What effect are the patient's medications having on the plan? • Is the patient's nutritional status appropriate?
