N104: Essentials of Nursing Care – Health Safety Study Guide 3.4 The Nursing Process and Communication Issues The nurse uses the nursing process to evaluate the patient’s communication and learning needs. Assessment The nurse must collaborate with the patient to assess the patient's current knowledge level and what education needs to be provided. It may be helpful to talk to the patient, family, or caregiver to determine the way optimal learning occurs for the patient. Does the patient learn better by demonstration? Or does she want written instructions? Analysis and Diagnoses During the analysis and diagnosis part of the nursing process, the nurse and patient may identify the following as communication diagnoses: • Impaired verbal communication, related to decrease in circulation to brain as evidenced by difficulty in speaking/slurring of words/difficulty naming words • Anxiety related to impaired communication, as evidenced by difficultyin speaking/slurring of words/difficulty naming words • Self-esteem disturbance related to impaired communication, as evidenced by difficulty in speaking/slurring of words/difficulty namingwords • Feelings of isolation related to impaired communication, as evidenced by difficulty in speaking/slurring of words/difficulty namingwords During the analysis and diagnosis part of the nursing process, the nurse and patient may identify the following as learning diagnoses: • Knowledge deficit related to new diagnosis/complex treatment • Non-compliance with treatment goals • Altered health maintenance related to patient's lack of knowledge or skills Planning During the planning phase the nurse, patient, and family will decide what needs to be taught and the best way to teach it. The nurse must consider all the factors that influence communication and learning as discussed above. The nurse develops the teaching plan with measurable learning objectives. What will the patient learn? How will the teaching be provided? How will we know that the teaching is effective? The plan must be centered on the patient's educational and communication needs. During the entire planning process the nurse must also be aware of nursing standards, ethics, and protocols. The nurse must also be sure to include other team members in the education planning process. Some educational efforts may be better done by another team member. For example, a physical therapist might be the best person to provide crutch training to a new amputee. Evaluation During the evaluation phase, the nurse must report and record the patient's responses by validating learning that has occurred. The nurse may verify communication and learning by:
