The pathways are assessed to see if they need to be modified as a result of quality improvement to help ensure that clinical variances are controlled and the pathways are used for evaluation of care, quality improvement within the organization, and cost analysis. The Physician Role within the Healthcare Team The physician provides medical care to the client, including disease diagnosis and treatment. The physician assists in selecting and activating the appropriate clinical pathway and collaborates with the team members of the inter-professional health team. The physician reviews the client’s progress and revises the plan of care if it is justified by review of patient outcomes. Other Members of the Healthcare Team • Therapists (physical, occupational, and speech) work with the client on activities of daily living, swallowing problems, ambulation, and home making tasks. • Nutritionists (dietitians) work with client to assess and monitor nutritional requirements. • Social workers work with the client and family regarding social structure, needs, and discharge planning. • Respiratory therapists provide respiratory treatment and pulmonary hygiene. • Chaplains work to meet the spiritual and religious needs of the client and family. • Physician assistants work with physicians and function under the supervision of an MD. • Nurse practitioners are advance practice nurses that can serve as a primary care provider and can diagnose and treat clients. • Certified nurse aides assist the client with feeding and personal hygiene need. Quality and Safety Education (QSEN) Teamwork and Collaboration QSEN defines this competency as functioning effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care. The newly graduated RN should be able to describe his or her own strengths, limitations, and values as a teammember and should be able to describe the scopes of practice and roles of other health team members. He or she should be able to analyze differences in communication style preferences among team members, patients, and families and describe effective strategies to communicate and resolve conflict. Achieving this competency also means that the new RN understands the impact of team functioning on safety and quality of care, as well as being able to describe barriers and catalysts of effective team functioning. TeamSTEPPs TeamSTEPPS (Team Strategies and Tools to Enhance Performance and Patient Safety), is an evidence- based teamwork system designed for health care professionals by the Department of Defense’s Patient Safety Program and the Agency for Healthcare Research and Quality. Teams make fewer mistakes than individuals, especially when each team member knows his or her responsibilities, as well as the responsibilities of other teammembers.
