N209: Reproductive Health Study Guide Implementation • Initiate nursing plan of care to utilize interventions that will move the patient toward expected outcomes. Evaluation • Evaluate the patient’s response to nursing care, and revise the plan of care as needed to meet expected outcomes (i.e., refer to breastfeeding resource center if patient is having difficulty with feeding, provide patient and significant other with information regarding parenting/sibling classes, etc.). Nursing Diagnosis Pertinent to the Postpartal Period • Risk for bleeding related to retained placenta, uterine atony, and cervical tea • Impaired urinary elimination related to regional anesthesia and extensive perineal repair • Deficient knowledge related to postpartum care and newborn care • Interrupted breastfeeding related to prematurity of newborn, congenital anomaly, or maternal complication • Risk for infection related to retained placenta, mastitis, C-section, or PROM
