N209: Reproductive Health Study Guide • Identify nursing interventions to help the patient achieve expected outcomes (smoking cessation measures, no alcohol, dietary changes, providing knowledge of risks, and diagnostic testing). • Consider patient’s access to health care and response to congenital anomalies. Implementation • Initiate and complete nursing care plans utilizing nursing interventions identified to develop a relationship with the patient and promote prenatal health and education. • Provide referrals for genetic counseling as needed. • Educate the patient and family on care of the child with a disability. Evaluation • Assess the patient’s response to nursing interventions and progress in meeting goals. • Revise the plan of care as needed to meet the needs of the patient and the family in provision of care depending upon the anomaly (educate on lung sounds, proper feeding methods, and stress the importance of medications). Nursing Diagnosis Pertinent to Congenital Issues • Ineffective infant feeding pattern related to cleft lip/palate • Deficient knowledge related to caring for a child with a congenital anomaly • Risk for decreased cardiac tissue perfusion related to aortic stenosis, patent ductus arteriosus, ventral septal defect, etc. • Grieving related to fetal condition incompatible with life
