Hypochondriasis This is marked by a persistent fear or belief that one has a serious illness. The phobia is an intense, irrational fear of something external. Fear persists even though the patient recognizes its irrationality. Phobias are characterized by episodic anxiety in response to specific precipitants, such as flying, talking in public, and insects. Signs and symptoms include panic when confronted with the feared object and persistent fear of specific things, places, or situations. Treatment: individual therapy; systematic desensitization; anti-anxiety medications; MAO inhibitors; tricyclic antidepressants Panic Disorder Panic disorder is a nonspecific feeling of terror accompanied by any of the following symptoms: choking sensation in the throat, racing heart, dizziness and fearfulness, rubbery legs, faintness, chest discomfort, or preoccupation with health and a feeling of going insane. Panic is an extreme level of anxiety. Panic attacks may not have a specific cause. 1.9 Pediatric Psychiatric Disorders Child psychiatric assessment includes: • Assessment is more time consuming in child psychiatry than in other branch of psychiatry or medicine; child mental health assessment is distinctive • It uses a developmental approach • All assessments, management, etc. must be related to child development (the normal attention span at different ages; how well a 5-year-old should read) • Take time to develop assessment skills of both younger children and adolescents • Be familiar with normal developmental milestones (motor, verbal, and social) and developmental assessments Psychiatric assessment includes: • Full history from parents and child • Mental state examination of the child
