N213: Health Differences Across the Lifespan 3 Study Guide Signs and symptoms of inguinal hernia: • Lump in groin • Pain relief if hernia is reduced/pushed back Nursing assessment for inguinal hernia: • Inquire about history of reflux and heartburn; this is the most common symptom • Take a thorough history of patient’s activities including exercise, lifting, and straining • Listen to bowel sounds: if there are no bowel sounds, this could be a sign of strangulation • Palpate for swelling or lumps to groin area • Review laboratory and radiological data: o CBC: WBCs possibly increased o Lower GI x-rays Nursing diagnosis for inguinal hernia: • Pain related to bowel ischemia and displaced bowel in the inguinal canal • Activity intolerance related to pain, secondary to displaced bowel in inguinal canal • Altered gastrointestinal tissue perfusion related to compression of bowel tissue and compromised circulation to bowel area • Knowledge deficit related to the disorder and treatment plan Nursing interventions for inguinal hernia: • Assess pain and give analgesics per MDorder • Teach non-pharmacological pain management: relaxation, diversion, and visualization • Offer warm blankets, back rub, and warmbath • Prepare patient for possible intervention: o Reduction: The physician may move the herniated area back into place. A binder or truss may be placed to hold the area in place to prevent reoccurrence. o Hernioplasty: The surgeon may place a steel mesh or wire in place to reinforce the area. o Herniorrhaphy: The surgeon surgically moves the herniated contents back into place and seals off the opening. o Bowel resection/hernia repair: The herniated bowel is re-sectioned and the herniated area is repaired. • Place the patient head down and feet up to reduce any pressure on the lower groin region • Teach the patient to try and avoid coughing and splint the abdomen • Teach patient to deep breathe or use incentive spirometer • • Swelling at inguinal area Sharp pain in groin area
