Meconium Aspiration • Hypoxia due to physiologic stress in labor (cord compression or placental insufficiency) that causes the fetus to pass meconium in utero • Aspiration of lung fluid can cause lung injury and RDS • Diagnosis is made via meconium noted in amniotic fluid with respiratory distress; CXR for confirmation Fetal Spectrum Disorder (Fetal Alcohol Spectrum Disorder) • Physical and behavioral problems associated with alcohol use in pregnancy • There is no known safe amount of alcohol in pregnancy • Can lead to abnormal facial features, small head, shortened height, hyperactivity, poor memory and learning disabilities, lower IQ, visual/hearing problems, and heart/kidney/bone problems • No cure; early intervention can improve development 5.20 The Nursing Process Assessment • Assess and gather information regarding the patient’s current health status, including obstetrical and health history, family history, past and current labor history along with pregnancy complications and risk factors and delivery information. • Assess influencing factors, such as socioeconomic status, religious beliefs, cultural factors, and age. • Review diagnostic information, such as prenatal lab results. Analysis/Diagnosis • Identify actual/potential problems. Create a nursing diagnosis relevant to patient status (i.e., acute pain related to perineal repair, deficient knowledge related to baby care and feeding). Planning • Establish patient centered goals with patient, family, and team members to achieve expected outcomes for patient regarding health restoration and health promotion (i.e., patient will learn postpartum activity restrictions, diet, perineal care, etc.). • Establish rationale for interventions based on evidence-based practice guidelines and standards of care, taking into consideration legal and ethical implications pertinent to patient situation (i.e., adoption, social service referral, religious beliefs, and childrearing practices in culture).
