Implementation As interventions are implemented, the nurse must constantly reassess the individual and family to ensure that the interventions are producing the desired outcomes. Safety of the individual must be the first intervention and the nurse must ensure that the family understands modifications that must be implemented and changes that should be reported. The nurse should be sure the interventions are culturally congruent. The nurse must establish a rapport with the patient by respecting cultural and individual differences. An interpreter should be used to facilitate communication if there is a language barrier. The nurse may help the family obtain needed financial and other social support. Evaluation The nurse caring for the culturally diverse patient must continually reassess the patient and family to determine the effectiveness of interventions. When an individual does not comply with a particular part of the plan of care, the nurse must recognize that this non-compliance may be a result of differences in culture. The responses must be documented and shared with other healthcare team members as appropriate. If the intervention was effective, it should be continued and expanded as appropriate. As always, the nurse is responsible to determine the patient’s response to care provided by another member of the healthcare team.
